creating multiple copies of the same document.

• Remember: Each Document Coversheet includes a case-specific barcode that is

used to attach the documents to the electronic case file. Do not copy or use a

Document Coversheet for client(s) who are not associated with that case.

• If submitting an application and supporting documents by mail, be sure to remove all

paper clips and staples.
